- Finding
- The official wartime Navy log inventory lists 'LCI(L)(G)-565' with log coverage 'Mar. 3, 1944-June 30, 1945' in 2 logbooks - the start date coincides with her 3 March 1944 commissioning and the end date brackets her documented combat service through the Okinawa operation (26 March-28 June 1945). Establishes in-scope commissioned service and points to NARA-held deck logs for the hull.

G. J. Headley USN in command (May 1945); reclassified Landing Craft Infantry (Guns) LCI(G)-565, 15 June 1944. Assigned Asiatic-Pacific: LCI Flotilla Six (CDR A. R. Montgomery USN), LCI Group Sixteen (LCDR R. S. Richabaugh USNR), LCI Division Thirty-Two. Campaigns: Leyte landings 20 October 1944; Lingayen Gulf landings 9 January 1945; Okinawa Gunto assault and occupation 26 March-28 June 1945. Three battle stars; ribbons include Philippine Presidential Unit Citation and Philippine Liberation Medal. Decommissioned and struck, dates unknown; sold 18 November 1946; fate unknown. Spec block: 246 t. light / 264 t. landing / 419 t. loaded; 158'5.5" length; 23'3" beam; propulsion two sets of 4 GM 6051-series-71 diesels, 4 per shaft, GM reduction gears, twin variable-pitch propellers, 2,320 shp, two 30 kW 120 V DC diesel-driven generators; LCI(L) complement 4 officers + 24 enlisted, LCI(G) 5 officers + 65 enlisted. - NavSource is a volunteer secondary compilation whose ship histories are credited to the USS LCI Association's Turner volume - milestone dates track official records (NARA log coverage corroborates the commissioning date) but it is not a primary registry. Contains no asbestos content. 'In command (May 1945)' dating for Headley conflicts with Lt.(jg) J. A. - The ship's own Okinawa action report, signed J. A. Whelan, Lt.(jg), USNR, Commanding Officer (documenting the post-Headley command): staged from Leyte 25 March 1945 in Tractor Group Easy of Task Force 51. On 1 April she preceded the first wave of LVT(A)s to White Beach, western Okinawa, firing two rocket salvos of 120 rounds each at 1,100 and 700 yards plus 40mm/20mm fire, with LCI(G)-422 and LCI(G)-77 on her beams. Then: anti-small-boat patrol off southern Okinawa (3-19 April), smoke-screening stations at Kerama Retto (19 April-6 May), patrol station 101 (7-21 May), anchorage smoke screening (22-27 May), patrol stations 105 and 104 (28 May-17 June), patrol to 20 June. On 4 June she searched unsuccessfully for survivors after USS Mississippi (BB-41) and USS Louisville (CA-28) were hit by suicide planes; on the night of 16 June she joined the survivor search for the burning destroyer identified as DD-591, USS Twiggs, alongside LCIs 650, 567, 422 and LCS 114, continuing to 0020 on 17 June without locating survivors. Ammunition: 230 rockets, 680 rds 40mm, 504 rds 20mm. No material damage. First endorsement 13 July 1945 by Commander TG 52.17 (Commander LCI(G) Flotilla THREE) - operationally under Flotilla Three at Okinawa while administratively Flotilla Six/Group Sixteen (copies to ComLCIFlotSix and ComLCIGroupSixteen).
